Dressings, Surgical Sets, Personal Protective Equipment, and Disposable Linen

Supply of dressings, surgical sets, personal protective equipment, and disposable linen: Detailed scope, form, capacity/volume, dose, unit of measure, quantity per package, and number of packages are specified in the Assortment and Price Form (Appendix 1 to SWZ). Medical devices must be approved for marketing and use according to the Medical Devices Act of 07.04.2022, possessing relevant certificates or declarations of conformity. Surgical products must have sterility class II A rule 7. Delivery is based on current needs, within a maximum of 2 working days from the order date, to the hospital pharmacy warehouse of Wojewódzki Szpital Specjalistyczny im. J. Gromkowskiego. Batch and expiry dates must be indicated on individual packaging and invoices, with a minimum expiry date of 12 months from the delivery date. Information on subcontractors must be provided with the offer, if known; in case of a change of subcontractor whose resources were used to meet participation conditions, the bidder must demonstrate that the new subcontractor or the bidder itself meets the requirements to the same extent.
