Provision, implementation, and operation of a cloud-based Software-as-a-Service (SaaS) solution for continuous digital discharge and transition management, including optional AI-supported extension modules. The solution coordinates the entire discharge management process, from early discharge planning and case-related coordination of follow-up care to the creation of discharge letters, across sectors, prevents care gaps, and reduces administrative efforts in the hospital. The system must comply with the requirements of the Hospital Future Act (KHZG), ensure legally compliant documentation and interoperability with other systems, and support KHZG-relevant digitalization projects through the integration of modern technologies, process automation, and improved communication.
