Implementation and delivery of a system for a Regional Digital Medicine Center (RCMC). The contract template is Appendix No. 4 to the SWZ. Required qualifications include evidence of economic and financial standing (e.g., liability insurance), technical and professional capacity (e.g., references from the last 3 years, personnel qualifications), and grounds for exclusion (e.g., extracts from the National Criminal Register and commercial register). A certificate as per Art. 124 sec. 2 of the Polish Public Procurement Law (PZP) may be submitted. A completed and signed European Single Procurement Document (ESPD) must be submitted with the offer.
