Supply of disposable products, divided into 18 packages, for patients of SZPZOZ im. Dzieci Warszawy in Dziekanów Leśny. The detailed scope of delivery is described in the tender documents, particularly in Annex 2 (assortment and price form). Bids must meet minimum requirements and include relevant evidence for medical devices (current document confirming market authorization and use as a medical device according to the Act of 07.04.2022 and EU Regulation 2017/745 (MDR) / 2017/746 (IVDR) with CE marking, if applicable, a certificate from a notified body, a declaration regarding the notification/registration of the medical device with the URPL) and informational materials as per Annex 2. Supplementation or clarification of evidence is possible.
