Supply of medicines for Megrez Sp. z o.o. Hospital: Prices for reimbursable medicines, foodstuffs for special nutritional purposes, and medical devices must not exceed the maximum prices set by Art. 9 para. 1 of the Act of May 12, 2011. Detailed specifications are provided in the assortment-price form (attachment no. 2 to SWZ). Execution is according to contract provisions (attachment no. 4 to SWZ). The tender is divided into 15 packages; offers can be submitted for at least one package, with each package to be priced in full. In the assortment-price form, only one product name and manufacturer should be provided per item, unless otherwise required.
