Supply of medical gases, including rental and refilling of cylinders and tanks for the locations Krakow and Maków Podhalański. The minimum shelf life/warranty period is 12 months from the delivery date, excluding liquid medical oxygen. The stated gas quantities are estimated, based on consumption over the last 24 months. Medicinal products must be approved according to Polish pharmaceutical law, medical devices according to Polish medical device law. A site visit to the delivery locations is recommended. Technical gases are subject to a separate lot.

Supply of blood collection and analysis kits to the contracting authority's headquarters (os. Złotej Jesieni 1, Kraków) according to Annex 1 of the specification. The minimum shelf life or warranty period is 12 months from the delivery date. Training of personnel at the contracting authority's headquarters (os. Złotej Jesieni 1, Kraków) is to be provided upon request with the first delivery to the Hospital Pharmacy, no later than 30 days after contract conclusion. An attendance list for the training must be submitted to the Hospital Pharmacy within 5 working days. The contracting authority reserves the right to request samples for testing; associated costs are borne by the contractor. Further conditions are specified in the contract draft (Annex 3).
