Training and Support Services for Coordinated Primary Health Care
Training and support services for coordinated primary health care within the project "Coordinated Care in Primary Health Care": analysis and evaluation of the implementation of coordinated care solutions in primary health care, hybrid and online training on coordinated care, diagnostics, treatment, and prevention, two-day on-site training visits at primary health care entities focusing on practical aspects of implementing coordinated care, and coaching after the training. An option for up to 30 additional training sessions under the same conditions is included. The project is co-financed by the European Social Fund Plus.
