Delivery of disinfectants for the USK-2 PUM Clinics: Estimated quantities are detailed in the unit price form (Annex 2 to SWZ, FCJ). Required products include medicinal products (Task 3, positions 6,7,8,9,10,17) according to Pharmaceutical Law, biocidal products (Task 2, positions 1,2,12; Task 3, positions 2,3,4,5,16,18) according to EU Regulation 528/2012, cosmetic products (Task 3, positions 1,11,12,14,15) according to EU Regulation 1223/2009, and medical devices (remaining tasks) according to the Medical Devices Act. For Task 8, a food contact certificate is required. The minimum shelf life is 12 months from delivery, except for Task 2, position 1, which requires 24 months. For Task 10 (cleaning and disinfection of endoscopes under warranty), no equivalent products are permitted; after the warranty period, equivalent products may also be procured. Quantities are estimated and can be reduced by at least 60% or increased, provided the total contract value is not exceeded. The tender is divided into 10 lots. Contract duration: 24 months from contract signing.

Delivery of various medicinal products, medical food, and nutritional preparations for the clinics of USK-2 PUM in Szczecin: The detailed description and estimated quantities are included in the unit price form (Appendix 2 to SWZ). Medicinal products must be offered in accordance with the Pharmaceutical Law of September 6, 2001, with the exception of special medical food for task 9 item 43 and task 13 items 1-3, and medical devices for task 10 item 3. Medical devices must be placed on the market and used in Poland in accordance with the Medical Devices Act (Dz.U. 2024.1620). The minimum shelf life of all offered products is 12 months from the date of delivery. For trade names, offering equivalent products is permissible, provided they meet the requirements for active substance, dose, form, route of administration, and packaging size. Equivalent medicines are allowed if reimbursement by the National Health Fund (NFZ) is possible. The delivery of medicinal products must be labeled and secured in accordance with Commission Delegated Regulation (EU) 2016/161 of October 2, 2015, including a unique identifier (UI) and an anti-tampering device (ATD) for prescription and certain over-the-counter medicines released after February 9, 2019. For discontinued or temporarily unavailable products, the position will only be evaluated for bidders who have sufficient stock for the entire contract period. The contracting authority may order a smaller quantity, but not less than 60% of the quantity specified in the unit price form. The order is divided into 15 tasks; further items as per the tender documents.
